<p>The end of the last page on the MIT application says "I have completed this form accurately...etc."</p>
<p>I submitted online, and when I print out the application, my name isn't on it! What the heck! I can't believe it accepted the application without that. But I remember seeing my name in the box. Maybe there was another??? They're not picking up the phone either.</p>
<p>I thought there was a note somewhere on the website saying that your signature doesn't appear on the preview. I think that happened to me too, don't worry about it too much.</p>

<p>I should clarify; I accidentally didn't sign the form (have no idea how they let me submit Part II w/o it) and that's what the rep I emailed told me to do. Alas and alack, I don't seem to have that email any longer. Can try to get that confirmed separately, though.</p>
