My son became interested in JHU years ago when he became involved with the CTY/SET program at JHU. Will this help his chances? He is a strong candidate as well.

Johns Hopkins does like to see that students have attended CTY programs or the JHU pre-college program, but it does not provide a student with an added advantage in the admissions process. More than 40% of our applicant pool have been connected to JHU through such programs.

I will say though that having done academic enrichment programs like CTY, can never hurt a student's application and is actually something we look for when reviewing a student's work.

SET sends out rec letters upon request, supposedly to show that the applicant achieved a high test score back in middle school.
What do adcoms think?

Standard Letters of rec from a program like SET (esp. related to middle school years) will not play any role. If it was a rec letter that was not general but actually spoke of the individual students and their performance, then that would play a role.
