<p>I think I printed them from online. Took son in for a physical and to get the forms filled out. He came away with 2 shots.</p>
<p>I downloaded the immunizations form from a link on an email Vanderbilt sent and then I faxed them back once the Dr. completed them. I also filled out the health insurance waiver form on line at that time. Shortly after we received an email of confirmation that the waiver was approved. I believe the links were on the same email but you can find the forms on the student health page of the Vanderbilt web site.</p>
<p>Just want to mention for those of you waiving insurance, to tell your child that if they go to see a specialist or to the ER at Vanderbilt, to have them bring their health insurance card, or you will have a lot of bills to sort out. And if your child is over 18, they may need to be the ones calling the physicians offices to sort them out. You can’t do it for them because of HIPAA, unless they call first, give permission to speak to you and hand the phone over or something like that. (Student health is no problem even if you’ve waived. They are still eligible to be seen there, but the clinic closes early, like 4:30 or 5 pm I think.)</p>
