if so, what did the letter your counselor write look like?

Is it enough just to say something like:

Dear Admissions Office,

After looking at (name of applicant)'s current circumstances, satisfying application fees would put considerable financial pressure on the applicant. On behalf of the applicant, I request a fee waiver for the application fee.

It's not really a letter (at least the ones i got). Some are letters though.
It's a form kind of thing from the college board. It looks like SAT waivers but wider and shorter. It also has a code number to it so in case you're applying online and the counselor has to sign it if applying on paper.

If you're low income or get free/reduced lunch at school, you qualify.
In one college (i don't remember the name), your family has to have 20k income or less.

If it's an emergency financial situation, explain it to your counselor and have her express it in detail on the letter.
that letter above doesn't explain anything. anyone could have written it.
Also, make sure you have record of proof of financial situation.
Also, you should send an email to them directly explaining a little
It probably isn't as complicated as this. let's hope it's not.

also, some colleges waive fees if you apply online or attend one of their info sessions or suscribe to their mailing list e.t.c
