<p>Yes, the teacher eval forms in the common app are the exact forms that your teacher recs should be sent on. Teacher recs are sent through the common app using the teacher eval form. If your teachers have used a different format to write the rec, that’s fine, they can just attach or upload a separate letter of rec to the teacher eval form and submit that whole thing.</p>
