Do I need to put anything specific on supplementary materials so they know who I am?

I doubt there would be another person applying to MIT with the same name as me, but I was wondering if there is anything I need to attach to an extra essay I want to send and an extra teacher recommendation so they know to file it under my name?

Also, is it better to fax, email, or mail these materials? Should I do all three just in case?

Just put your full name and your birthdate on any supplemental material you send, and it'll be filed in the appropriate place.

I would vote for faxing or mailing over emailing -- emailing requires admissions staff to print out your material, and is something of a pain.

You also can't send any additional materials in an attachment - they need to be copied and pasted directly into the body of the e-mail. I would suggest faxing - it seems to be the easiest and the quickest thing to get processed.
