Do schools want the secondary school report form?

<p>I'm applying to Brown, Penn, Cornell, and Hopkins. Brown says that it wants a "school report form" and the rest of them say that they want a high school transcript. Do most schools want an actual high school report form with comments from the guidance counselor? I don't know if my high school has old report forms on record, or if they would need to create a new one for me (which would be a serious problem if my guidance counselor no longer works there). </p>

<p>Or is the high school transcript enough? Assuming it shows my class rank and GPA, it should be enough.</p>

<p>Today, I just asked my high school guidance counselor about such issue. (I am applying Cornell) She said those schools just want your high school transcript. The common application form for high school doesn’t very matter. Once you send the official high school transcript, you are ok. That is what she told me today. Brown maybe a little different, the others should be enough with a offical transcript.</p>
