Doc, I have a question

<p>I just recieved in the mail today a little card that says the AF academy is missing my Personal Data From 146 in my file. This was one of the “bubble sheet” forms that was sent with my candidate kit. The frustrating thing is that I know for sure that I sent it to them, along with the other 4 or so bubble sheets they had me fill out. And, they have recently sent me similar cards like that and never have they said that this particular item was missing, and this was after I had already sent in all of the candidate kit requests. It doesn’t make any sense. </p>

<p>Now I don’t exactly know what to do because I don’t exactly have a blank personal data form laying around. Do I need to write them for another one? Any advice would be great. Thank you.</p>

<p>Make sure they don't have it by using the automated telephone # before bothering to request an additional form. 719-333-3333</p>

<p>It is probably too late for this advice, but we were told to keep copies of everything we send, and to send everything return receipt.

My daughter has a complete set of blank app forms sent her by the afa before she made the decision to only pursue the naval academy and west point. We can mail one of the forms to you if you need it.</p>

<p>I had the same EXACT problem. I got a postcard last week in the mail saying that I was missing three things...the same personal data form as you, PLUS both teacher recommendations! I checked with the teachers and they said they sent them over a month and a half ago. I called the counselor at the Academy, she said that they really did have the first form 146, but still did not have the evaluations. THEN, after telling her when the teacher's sent them, she came back emailing me to tell me that they indeed did find ONE evaulation from one teacher. What goes on over there? I would love to be a fly on the wall...
All the while, the phone admissions number tells me that all I am missing is a nomination. Go figure. Is this character building or what? Anyway, the woman at ASAFA is sending me another eval. form for the one teacher that they didn't get. What a pain, huh?? So....you're not alone, just thought you might like to know! Maybe this is test??? :)</p>

<p>I can understand the nomination thing on the automated system..because I definitely haven't even had my congressional interviews yet. But it says that everything is in on the automated system as far as my application goes. Yet, they are obviously intent on sending out that little note card. They either need to get some stuff straight over there, or keep on doing this test, because I'm sure it frustrates enough people to get a good reading... But i won't let up, i'm getting there. </p>

<p>If you got a card that said you were missing the personal data form 146, and then you called your counselor and she said that she had it...What excuse did she have for the sending of that card in the first place?????</p>

<p>Hey, team, snail mail and the admin behind it does not travel at the speed of light. I would go first by direct word of mouth by the admissions counselor. Then, get their name and make of record of your conversation. Ask them to Fax or e-Mail a note confirming your file is complete. Don't hesitate to bug them if you're at all concerned--that's their job. The cards are notoriously late--talk to a real person to confirm.</p>

<p>She gave no excuses or explanations...just the facts! I didn't want to egg her on and ask either. Didn't know what was the correct etiquette...</p>

<p>Be respectful but persistant. Remember, the thing they fear most is a call from a congressman's/woman's office asking why his/her nominee's package isn't being handled properly and promptly. Don't worry about making their life difficult. They're there to serve USAFA and you.</p>
