<p>USC requires one recommendation, and it may be from a counselor OR a teacher. If you would like to submit both a counselor recommendation AND a teacher recommendation that is fine - both will be accepted and considered.</p>
<p>Yes, the “Counselor or Teacher Report” is the correct form. In the past it was specified as “Form 4.” It can be used for both the counselor and the teacher report - just print out two copies.</p>
<p>It seems “most” applicants choose to submit both a counselor report and a teacher recommendation, and some choose to sumbit three recs - a counselor and two teachers. As long as you have at least one it will fulfill the requirement.</p>

<p>You “can” send whatever recommendation you want to send and USC will read it. </p>
<p>Universities in general would like recommendations from individuals who can comment on you as a student and who have *direct knowledge *of your academic capabilities and performance. If you would like to send a recommendation from an alumnus who has not been your teacher or counselor, you should send two recommendations - with at least one being from a teacher or counselor.</p>
