<p>Perkins Promissory Note-</p>

<p>It's basically the contract that you HAVE to get signed and turned in to get your Perkins Loan. If you don't have it yet, then get it. It is not available electronically and the Financial Aid Office at Berkeley only accepts paper form. If you have other Federal Direct Loans, you should have completed a Master's Promissory note online and completed the entrance counseling exam, which you have to take online and print out results that show you've passed it.</p>

<p>Yup, that's what I did, that's what they asked for at the bottom link in Bearfacts. I also asked the counseling service to send the results electronically to Berkeley, but I also printed out the "Rights & Responsibilities" page and sent it via mail to Cal as well (just in case, did it two days ago).</p>

<p>the "checklist" that they told us to send. It's not really a form, right? more of a list that has our rights and responsiblities. So do we just sign in the bottom space and send it back?</p>

<p>YEA! that really screwed me up i was scrambling to look for a "form" or a "checklist" and all i had was a stupid list..so i guess i'll just send that in. i know theres another site for the MPN, do we have to do that too to get our loans? thanks.</p>
