do you put in the upper left hand corner:

dob
school name

even the short answers?
should you number the essays/include the essay question?</p>
<p>Generally the school should specify the format they want. But generally in a header put your full name and your social security number. I'd also number the pages if there are more than one. And I would include the question at the top of the first page, and the number or letter if applicable.</p>
<p>You could put all the short answers on the same page- depending on length. That would be fine. A 10 sentence paragraph doesn't need a separate page.</p>
