<p>Written comments that elaborate on you as a student are certainly preferred by Common App member colleges. Whether those comments come in the form of a separate letter or notes on the evaluation form itself is generally not of concern to colleges.</p>
<p>The two teachers who wrote LORs for my S completed the form, and then provided a personalized, one page typed evaluation on HS letterhead, with their signature. It was very professionally done and appreciated by all.</p>
<p>The only things they didn’t fill out on the form (the both of them) were the HS CEEB Code and class rank, which were easily available from the GC.</p>
