One of the things that I have to bring for Explore USC (for merit scholarship finalists) is “Signed Parental Consent and Student Health Insurance Form.” However, when I was signing up for Explore USC, I did not find parental consent form.
Where can I find the form??
I have Explore USC program in two days, so please help me!!!
