<p>I wouldn't do that because it's not a good idea to give people your SS #. I'd just make sure that the recommenders include your full name and high school and what you're applying to (e.g. freshman entrance).</p>
<p>I would put the same identifying information that is put on the other recommendations. Indeed, what my daughter did was to use the same form but put "supplemental" at the top of the page.</p>
<p>Sometimes schools tell you specifically what identifying information should be put on supplemental information or any extra pages. I've seen places that ask for SSN, address, or birthdate in addition to the full name. I wouldn't put the SSN unless you are specifically directed to do so.</p>
