<p>I have a rec from a person that I interned for.</p>
<p>I did not give them the official school form for the schools I applied to, but I gave him envelopes w/ stamps and addresses. Is it okay if he just sends them w/o the formal form (since he isn't an academic teacher) but in the letter he clearly states it is a rec for me?</p>

<p>I agree with whatever: As long as it is the person who directly supervised you during your internship, and he will be adding new and important information about you beyond what the teachers and school rec's provide, one supplemental recommendation is fine. However, double check with each school before sending as there are a few schools who frown on receiving supplemental recommendations and will in fact not consider them.</p>
