Extracurriculars for residency/fellowship

I am a pharmacy student in a 6 year program. I was trying to get some really interesting extracurricular activities added to my CV. Right now, I do research, have joined various organizations, and have an on-campus job at residence halls as desk attendant. I am interested in doing all of that. I haven't had any papers published though..I intend on doing research with a different professor in the pharmacy department of my school next year though.( The professor keeps changing experiments on me. they stay relevant but we never finish anything imo..)

I had desires to do certain things but then I found out how long term they were..one of them is 8 hrs in one day every week. The curriculum in my school is very intense and I know that there are ppl who stdy a lot, have no ecs, and are barely in the program. I am worried that I might fail out if I had joined these ecs so I decided not to. I just wish everything wasn't a "decide now for what you will do for the next 3 years" thing. And I don't want to do something just to put on my CV...it's so much internal drama and conflict. I decided not to be a peer mentor for something else and then I found out how much work it is..so now I have zilch. I am not very sure in what I want in life or anything else which is causing me all these problems.

The fact that I cannot bare to give a speech in front of people as part of the elections so that's out..

Why do you think you need any more activities?
