<p>I called CSS on this, because I wanted one too…So, I think if you’re just coming out of high school, your school counselor has it. But, if you’re already in college, you have to ask the school directly. </p>
<p>In my instance, I wanted to apply to USC. I emailed their financial aid office, they sent the form to me in pdf and gave me a fax number so I could send it in to the school directly.</p>
