<p>My counselor simply just wrote a note to attach to my application saying that I needed a fee waiver...</p>

<p>so i was wondering if i am missing some sort of official form for a waiver?</p>

<p>i mean, it seems too simple...</p>

<p>I think you should call the admissions office and ask about additional forms. It does seem a little to simple to attach a handwritten note.</p>

<p>my counselor gave me an official CollegeBoard fee waiver slip. In order to use the slip you need to have used a fee waiver for at least one SAT or SAT II test.</p>
