<p>There are shareware-type programs out there that allow you to fill in the blanks on a .pdf, but they only work so-so. </p>
<p>Check out PDFill Form Filler at <a href="http://www.pdfill.com%5B/url%5D">www.pdfill.com</a> which costs about $20. I think they offer trial version that is what the industry refers to as "crippleware" because when you print the filled out .pdf, there is an annoying banner/watermark on the sheet. The watermark disappears when you register and pay for the software.</p>

<p>PDFill Form Filler is actually very popular and gets good end-user reviews, but read the pros and cons carefully to determine if the software's limitations are anything that you really need to be concerned with.</p>
<p>The software's performance is super slow on my old Win2000 laptop.</p>
