<p>Guidance counselor usually takes care of it. Certainly does at our school. Check with them I am sure you are covered.</p>
<p>my school requires us to request one. I have to bring the envelope with a stamp on it and the address because of high costs now a days.</p>
<p>I’m pretty sure there is no form UNC requires. Just have your guidance counselor send your transcript, and if you could include your PID somehow, that would probably be even better.</p>
<p>There was a required Final Transcript Form last year, which had to be sent in with the official school transcript. I found it on the UNC website, but it was dated 2007, (and there were some for previous years, too) and so far I haven’t found one dated 2008, so maybe it’s not required this year.</p>
