<p>I couldn't find a document to fill out for the final transcript like that of the common app. So, I was wondering how we would report our final grades? Do we just send a transcript without any attached form?</p>
<p>I had the exact same question and emailed Columbia about it. This was their response: </p>

<p>Please send an official final high school transcript to the address below as soon as your final grades are available. We do not have a required form to accompany the transcript.</p>

<p>Actually there is a form. It should be in the package sent by the student advising department (which includes a page about the summer advising sessions and a brochure about choosing courses). Yes, but the form contains very little useful information: just your name, signature, and school info. So I guess they would accept the transcript mailed separately.</p>
<p>I believe the form epaminondas is referring to is a yellow form? At the very bottom, it said something like “If you have already sent your final transcript, please disregard this form.” So it is unrequired.</p>
