Forgot to complete supplement

<p>Okay today I was so stressed out because I submitted my Upenn application late by about 20minutes cuz like literally 1 minute before the deadline I realized I had make payments first. and then loading payment took forever :(</p>

<p>Anyways so I wanted to send my Duke application as soon as possible(few hours before the deadline lol) and quickly filled in the supplement</p>

<p>N I missed the instruction !!
"If you have lived outside the U.S., please attach a sheet listing the cities and countries in which you have lived, indicating the years of residence in each." and didn't attach anything
I am from Canada so I should do it...</p>

<p>Would it hurt my chances? (I didn't even write any optional essay)
Should I send the admissions office a separate document??</p>

<p>Contact your regional admissions officer, letting them know that you forgot to attach the sheet and that:</p>

<p>1) You attached the sheet to the e-mail; and
2) You’ve sent a copy of the sheet in the mail to the Admissions Office.</p>

<p>The admissions officers are really forgiving people - don’t worry too much about it. On that note though, make sure that you DO attach it to the e-mail and also mail another copy haha. Make sure that your e-mail and hard copy have identifiable pieces of information so that they can match the additional sheet with your application file (i.e.; include your application ID number, DOB, full name in the e-mail and on the hard copy of the additional sheet).</p>
