Form Code on AP Cancellation Form?

<p>Hi, I'm trying to cancel one of my AP exams but I'm having a little trouble with the cancellation form. Can one of you guys help me out?</p>

<p>What should I put for the Form Code part? It was there on the AP exam cover sheet but I don't remember it. I tried asking my counselor but since it's summer break she won't answer. Can one of you guys help me out on what to put there? Or for those who have experience in cancelling AP exams, is that part really mandatory?</p>

<p>I had the same question, but I asked my counselor, and she said that even she could not find the form code, since they’re on the booklets. I just put n/a on mine.</p>

<p>I don’t think the form code is really mandatory, since it’s not on any of the previous score cancellation forms.</p>
