Format for Activities Resume

<p>I was wondering, if I were to compose my own activities resume, what format should I follow? Do you know of any websites that can aid me in doing so? Also, if I were to attach this resume separately from the Personal Statement form on the application, what information should I include on it to let the admissions officers know that it is indeed typewritten and attached separately?</p>

- activities/ volunteer work
- summer activities
- academic awards</p>

<p>you want your name, your school name, and either your social security number or your birthday.</p>
