Do extra recommendations (research mentors, bosses etc) have a specific format like the teacher evals? If not, then should the teacher evals be used as the giudeline?

If you are sending an extra recommendation, it can simply be a letter. Be sure anyone who is writing you an extra rec has something substantially different to say than those who've already written on your behalf. If they'll just be reiterating what's already in your file, it's not worthwhile. Think quality over quantity, definitely.

thanks for the info

each of my recs is in a different context so it should be ok

A letter is fine. Sometimes people xerox eval a or eval b and use the copy for a supplemental rec. Totally up to you.
