Forms!

<p>I am terrible at keeping track of all these random forms. Would somebody be so kind as to tell me what forms are already due and which ones are due soon?</p>

<p>I've never posted before, but since just days ago I found myself in a panic over the forms, I thought I'd give it a shot.</p>

<p>Most of the forms- NSO registration, the Meal Plan Contract, and optional pre-orientation programs- were due postmarked by June 10. The immunization form is due by July 1.</p>

<p>What about the GoCard form?</p>

<p>This page says to submit the form and then submit the photo at least 30 days before you arrive on campus so that it's ready by the time you get there.</p>

<p>You don't ahve to sub,it a photo early. they can take it there, but your GOCard will take alittle time, so it's up 2 u.</p>

<p>Is it really bad if the immunization form is late?? I just realized I have to do a tuberculosis screening and I can't get an appointment until july. I jsut don't want to pay the late fee.</p>

<p>No, not really, I sent mine in WAY late, because I live in Mexico, and the forms took long to get there, and they excused me of the late fee. They also told me that they won't usualy charge the late fee unless it doesn't get there before orientation. Why do you have to get a tuberculosis test? I had to get one, it sounds scary but it really isn't.</p>

<p>Because I was born in Russia and on the form it says that if you were born or resided in any country not on the list you had to get a tuberculosis test. Its probably going to take a while, because Russia provides some weird tuberculosis vaccine that shows up as positive for tuberculosis if you're tested for it in any other country, so I'm anticipating that that's going to take a while to clear up.</p>

<p>Speaking of shots...is that meninigitis (spelling?) one really necessary or is it just like a flu shot? Would you suggest the pain. My mom is worried that I'll fall deathly ill my first year of college but I would rather avoid any shots possible</p>

<p>As much as I am dismayed, yes. I have seen news reports which, even disregarding the intended dramatization, suggest that students living in dormitories should be inoculated. A google search on the issue - with the search terms being "meningitis" AND "dormitory" - should fulfill your curiosity.</p>

<p>I had the mengococcal innoculation (aka meningitis shot) a week ago and, as a professed wimp myself, it wasn't that bad. Your arm will be sore for a few days afterwards, but it really isn't that bad. I'd do it just to be safe.</p>

<p>I also had the mengococcal shot done a few days ago too (along with the chicken pox vaccine and the TB test on the same day) and it didn't hurt too badly, and my arm wasn't sore at all afterwards. (However, my perception of pain might currently be distorted cause i had ingrown toenail surgery recently, and any pain seems nothing compared to the pain of the shots they use for that.)</p>

<p>I took it, definetely take it, it is a really weird disease. It attacks your nervous system, and just getting it can leave you impaired for life. The interesting thing though, is that I'm from Mexico, and it has been erradicated here, so finding the shot was sooo hard. I took it once I got to the US (on campus), at the beginning of the year they ahve a campaign. If you get Gtown insurance it only costs 8 dollars, if I were you I would wait till I got there, unless you can get it cheaper.</p>

<p>however, there is a reason why the meninigitis vaccine is optional..and I believe that is because meningitis is a tad quirky (ie, you get vaccinated for x number of strains out of a constantly mutating bug...decently useless).</p>

<p>then again, it might be because there's like a one in a gazillion chance you'll get meningitis and die from it. or was that another one? oh wait that was small pox I think just disregard this.</p>

<p>yes there is, but the average person doesn't have to take the chicken pox vaccine. I never had the chicken pox and my blood tests show that i'm not immune, so i was required to take the vaccine. if you've had the chicken pox disease, you don't need the vaccine cause you're already immune.</p>

<p>Hey everyone! I wanted to ask..how do you sign up for the Gtown Health insurance? Also was I suppose to get some kind of packet concerning pre-registration, courses and stuff or is the stuff online all there is?</p>

<p>You get billed no matter what and then you decide wether you want to have it or not. You do that online, once u pay the bill.</p>

<p>pre-registration is online, for sfs at least. it wasn't a packet just a thing saying go here and sign up. there are instructions online, but I lost the address.</p>
