frosh app question: teacher evaluation = recommendation?

Does teacher evaluation mean the same as 'teacher recommendation'?
Is this different than the counselor letter? I notice the list of material noted below from UM's website does not mention the counselor letter.
How many teacher rec's can (and should ) be incuded?

What is considered a completed application?

Freshmen wishing to be considered for the Early Action deadline must have all required application materials postmarked by November 1st. A completed application consists of:

* The Common Application
* The U-M Supplement
* High School transcript
* Secondary School Report
* Teacher Evaluation

Office of Undergraduate Admissions: Early Action

The ‘counselor letter’ is the “Secondary School Report” (https://www.commonapp.org/CommonApp/Docs/DownloadForms/SSR_School_Form.pdf</a>)

A teacher evaluation is this https://www.commonapp.org/CommonApp/Docs/DownloadForms/Teacher_Evaluation.pdf</a>. (And isn’t necessarily a recommendation. Teacher is free to not recommend you for anything. Though I doubt that you’d want to go to a teacher who won’t.)

I would send in a maximum of 2 from teachers and 1 from someone outside of school if you participate in other activities.

thththanks JJJJUST. my son’s HS is not electronically plugged into common app. as I understand it, their teacher recs are the old fashioned free form documents written by the teachers. So the hs needs to make sure they fill out the common app pdf doc for both the counselor doc and the teacher doc.
