Good Operations Research program

Do I have a chance?

Majoring in Industrial Engineering, Minoring in math, Georgia Tech.
GPA 3.5+epsilon
All math and Industrial Engineering classes got A's in.
GRE Q:800, V:600
GRE math subject test: expecting >85%, I am good in math
Two very good recommendation letters from professors (did research with them, not published), a letter from my current job.
IE internship experience, no reference from there.

Do I have a chance to get into anywhere decent? Of course a PhD program.
What else do I need? Is there anything I can change before its time to apply?
Thanks,

(de-anon - I don't care)

I’m also looking at grad school in OR or a related field, but for MEng/MS. If it helps, here are the schools I will probably apply to:

Cornell
Columbia
Princeton
Michigan
Northwestern (for the PhD, b/c I worked with a professor there)

I’m sticking to the Midwest/East coast, but I think Stanford has a good program (Management Science I think is what the department is called) Berkeley and USC are other possible options.

I go to Cornell right now, and the OR department is really great. FWIW, an OR grad student I talked with at NU said the PhD program at Michigan is really cutthroat.
