I got ISS (in school suspension) after Early applications for academic integrity violation. I lost all my leadership positions for the school. Do I self-report ISS? Do I not check 12th grade for years of participation for my ECs? Would my counselor rewrite my recommendation just to include ISS? Would the coleges for my early applications contact the school and check?
This likely depends on the violation. You should speak with your guidance counselor directly to see what will and will not be reported to colleges in your recommendations. Definitely don’t misrepresent things in your application (i.e. claiming you have a leadership position you no longer have).

Your counselor may have already sent the letter of recommendation - but he/she will likely send an academic update with 1st or 2nd quarter grades. I do updated letters all the time, or place a call to the admissions office.
Honesty is the best policy. Report it, own it, apologize for it. Otherwise you risk having an acceptance rescinded. Read the fine print on your applications…I guarantee you that all of them have verbiage that states the information is correct and that if anything changes in relation to grades or discipline, you are obligated to let them know.
