I applied to Harvard and several other schools and harvard is the only one with this item missing I just realized this because I was logging in to all the app portals just to check, realized they are missing this item. Is it possible its still being processed? or am I just out of the running now 
I will be talking to my counselor on Tuesday during school about this.
I would speak with your guidance counselor and have them FAX your mid-year report ASAP.
Thanks for the reply @gibby but it seems that due to the way my school does things for everyone in my county it shows up like that. We use transcripts only and do not use the CA or UCA forms so that category is left unchecked.
@gibby This happened to me as well, my counsellor submitted my form through trackable mail and told me they received it last Thursday, yet on my portal it still shows as missing. Does the processing time increase at around this time?
My counsellor knows they received it because the trackable mail shows that it is signed and received by the office.
