<p>Did anyone else get the email today that says they have not received student's health forms and can de-enroll and won't let student into the dorm? If so, have you already sent your forms? I have proof D's was delivered there on August 12, three days before deadline. I called to see if there was some problem with what we sent but all they would say is I should fax the forms (glad I kept copies) and they would not have time to tell me if there was a problem with them.....do they lose everything? Already had to resubmit transcript and one other thing I knew we sent.</p>
<p>Same thing happened with me, it’s ridiculous.
I think they lose track and we just happen to be unfortunate ones. I faxed and mailed and mailed again…</p>
<p>S did NOT get that email, he but did get one that said that he was in compliance with New York State Public Health Law 2165/2167 (proof of immunizations, etc). We’re trusting that get’s him into the dorm on Move In Day.</p>
<p>I got that e-mail, too. I sent in my forms, but there was an error involving the date of one of my vaccinations. They told me to fax in proof that the date on the sheet was incorrect, so I did…twice. But today they told me that they can’t accept my proof, so I ended up just going to the doctor’s and getting a shot.</p>
<p>Long story short, the Student Health Center doesn’t seem to be very good with communication.</p>
<p>P.S. When I called them before sending in my second set of faxes, they said the due date for the forms had been extended to the 29th, which would make sense, seeing how that’s move-in day and all. Anybody hear more about this?</p>
<p>I mailed my forms a few weeks before the August 15th deadline and then on like August 9th they still had not recieved them so I went ahead and faxed them. Within like 2 days they said my records were on my account and ‘I was in compliance.’ I recieved an e-mail yesterday or today that said I was in compliance. I would recommend mailing and faxing the forms.</p>
