Health Form Question

Hi, I have a little question about filling in the Health Form.

Am I supposed to get vaccinated with all the vaccines listed in "Other important Vaccines" in Part III, or should my doctor just fill in the dates for the vaccines that I've taken. Also is that PPD Mantoux test required?

Thanks.

you are supposed to have all your vaccinations and yes its mandatory to have a ppd.

Are you absolutely positive? I'm not talking about the Measles/Mumps/Rubella vaccines on Part I, Section 2, I'm talking about the HPV/Hepatitis A and B/Varicella/Meningococcal/TDAP/Polio/BCG vaccines on Part III, Other important Vaccines.

Sorry, I just want to be sure before I get punctured with so many needles :P

Thanks.

I don't think you need those. I know they highly recommend the Meningitis one.

actually you are required to get the meningitis vaccine if youre not immune to it.

Section I - Requirements

<a href="http://www.health.state.ny.us/publications/2370.pdf%5B/url%5D">http://www.health.state.ny.us/publications/2370.pdf</a></p>

varicella is chickenpox... so chances are you prolly had it as a kid so youre immune already. and the others were requirements for high school so you should have all of them anyway.

Thanks, this helps.
