<p>did any incoming freshmen receive their health forms yet? it said on nyu's website that they will be mailed in mid may</p>

<p>I was referred to the following site to enroll or waive health insurance online:</p>

<p>Look on the Left Column towards middle of the web page where it says to click to enroll or waive health insurance.</p>

<p>I called the NYU health office and they said health forms are going out at the the end of june and should be filled out and returned by august 15th. There is no physical required, however immunization records need to be up to date and health history ect.</p>

<p>Hi, Sorry I responded to the Health Forms issue in an incomplete manner. Thanks to oreoegirl14 for making the call and providing the additional info.</p>

<p>I am now providing links to what is required and dates for completion. Please note from the above ^ that the health forms (health history and immunization records) will be mailed out to the students.</p>

<p>NEW UNDERGRADUATES STUDYING AT NYU IN THE USA</p>

<p>Student Health History form - Required
Student Immunization History form - Required
Meningitis Response form - Required
Alcohol and Other Drug (AOD) Health Module - Required
Student Health Insurance enrollment or waiver - Required</p>

<p>Link #2 from the NYU Student Health Center Online Letter to Parents: </p>

<p> Evidence of vaccination with two doses of the combined Measles, Mumps, Rubella (MMR)
vaccine, if not immune by history of disease or laboratory titer. </p>

<p> Completed </p>

<p> Submit a Meningitis Response Form to acknowledge that you have received information about
the disease as well as whether or not they choose to be immunized. </p>

<p> We ask that your student please complete the NYU required Student Health History and return
the required immunization information in the enclosed envelope no later than August 15th, 2010. </p>

<p> The deadline for completing Part 1 of AlcoholEdu® is August 31, 2010. The Part 2 deadline is
November 5th, 2010. </p>
