Health Insurance Waiver

<p>One of the 4 waiver criteria: "has a facility within 25 miles of the UCLA campus where full services are available." I have a HMO plan and that means I have to change the primary care physician for my daughter. I am from Northern Cal and I don't know any medical group nor doctors out there. As much as possible, I don't want to change physician but then how do u satisfy #4 criteria. Any students similar to my situation?</p>

<p>I'm from norcal too and I had to waive the ship thing. What I did was I went to my insurance company's webpage thing and looked up a doctor at the vicinity of UCLA, most were at UCLA med anyway. By that, I just selected a doctor. I never used my insurance during my year at UCLA so I doubt you'll have to worry about your daughter actually going to that physician.</p>

<p>Yeah that requirement means that there is a hospital, doctor's office, etc. within 25 miles of UCLA that accepts your insurance policy. If you have a PPO, you're automatically covered here by UCLA hospital and the Ashe center. If you have an HMO, you need to find one of those places. Doesn't necessarily mean you have to tell the HMO that you're switching physicians, but the one near UCLA has to be available for your son or daughter to go to if necessary.
Kaiser, for example, has a hospital just down the 10 freeway that satisfies this requirement.</p>

<p>yeah it doesnt mean you have to change anything. its just that should anything happen, you need to be able to see a doctor or goto a hospital that your plan covers within 25 miles of UCLA.</p>

<p>if I'm planning on buying the school's insurance, will it just be autimatically included in my fall quarter's fees?</p>

<p>I could've just put UCLA Medical Center? that would've saved me a lot of work. oh well, I have PPO, so I don't know why i did the HMO steps. And sorry, I don't remember the doctor I choose, it was pretty random. It was just one of the ones they listed at UCLA med. And to AWANG! haha, yes your fee will be included in your fall, winter, and spring quarter fees.</p>
