Well, I'm a freshman in hs looking to volunteer at a nearby hospital. Because my dad is a doctor, I'm basically in, but they require 2 recommendations.
So we had to fill out the application at the hospital, but at that time I didn't know they required recommendations. So, I just put down an English teacher who liked me in middle school, and a family friend.
The hospital is going to mail information about recommendations to my teacher, but I have no idea how to tell my teacher. I don't talk with her anymore, now that I'm in highschool, but I know for a fact that she did like me and thought I was a good student & writer.

I can only contact her via email, but I absolutely do not know what to write. Not only is it a bit awkward, I feel bad that I already put her down as someone they can get a recommendation from when I didn't ask her.
PLEASE HELP! Should I write some personal information (ex. how I'm doing in highschool) and how much before I ask her about the recommendations?

Applications to highschool [programs] are going on at the middle school, so she may be too busy to write one. If she declines, I don't know what to do!

Should I tell her to be expecting to write a recommendation or pretend I didn't already list her name and ask if she could write one?

Please help and give me advice about what to write and what I should do if she declines.
