<p>Yep, I would say so. The Questbridge team won’t revise your application without the form. You won’t be able to apply for the College Match scholarship, but you could still try to use your finished QB application to apply to your ranked universities through the regular decition process. I am submitting mine the same Monday 15. Good luck</p>
