I applied to MIT EA and I just got an email that they don't have my Evaluation B. They said I could fax it along with a coversheet. But where I do I find the coversheet on MyMIT? Do I just use the supplemental coversheet?

I have this exact same problem. I have no idea what happened to my evaluation B, it was mailed November 2nd. I think that they just mean the front page of the evaluation, not the supplemental coversheet, as that says not for use with the required evaluations.

they mean a coversheet that identifies the document and you (but don’t include your social security number! we don’t have it and we don’t want it! )

you can use either the supplemental coversheet or (if it’s EvalA/EvalB) the evaluation cover sheet - this works fine.

then just fax it in.

what if my teacher forgot to fax it with the coversheet?

should he resend it?

I’m still confused on what the coversheet is.

is it the first page of the eval. form in the link above?

^ Yes, that’s the sheet for evaluation B. If you want to resend eval B, send that with it :)

Thank you!

iceberg - probably worth it, just in case.
