<p>In answer to the first question: no, I don't think you need to include certificates. There should be a space on the form to list your extracurricular/high school involvement. If you want to emphasize these activities further (that is, if you consider them major accomplishments personally or academically) then you might write about or mention them in your essays or ask the people writing your letters of recommendation to mention them.</p>
