Help on research project

This is sorta related to college admissions, because research projects help with admissions. Anyway, I know on overachieving CC that this might be a stupid question, but how does a college research project actually work. I’m talking about the kind worthy of being put on an app. Does it have to be supervised by a professor? What format must it be in? What subjects can it be on? Thank you all.
