Help!!! Transfer application recommendations

I have a question about applications, specifically instructor evaluations...

I asked for a recommendation a month ago from my professor, but because of my Trimester calendar and scheduling, when he finished the letter was after spring break started. The only way he could get the letter to me was via e-mail, but there was no way to do this with the Common application forms; we tried to work it out but there was no other way.

Another thing is, he doesn't seem to want to fill out the forms too; he keeps telling me he is very busy, and he will write me the recommendation letter, but I should to the photocopying, and the forms. I won't see him until mid-April, either, and he seems to be officially "done" with my recommendation and I don't think he'll be too happy about being approached about it when he's busy.

Do you think it's okay if I send in the recommendation without the form? Or will they totally scrap my application as incomplete and not look at it at all? I'm really worried and asked the colleges, but no response so far... : (

I know at my school I had to give them addressed envelopes to all the schools I was applying and the professor was the one who actually mailed them out (photocopy of the letter and the common app form in each). Maybe ask him if you can mail him one of those big brownish envelopes and have all the little envelopes for the schools in it along with the common app form that he can fill out and make photocopies of. Then he can put his recommendation and the common app for in each one. I am pretty sure he has to sign it, so an email copy would kind of be pointless. That is really sad when a teacher says they are going to do a recommendation and end up pulling a half a$$ attempt at completing it. If he still doesn’t want to cooperate then I would first talk to the school you are applying to and if they don’t have an answer, then I would write a nice letter explaining why it is incomplete. You can also try asking another professor?
