Help with application!

Please help!
I'm trying to get everything done ASAP so I can go back to my school work!

1) I'm going to ask same teacher for early and regular decision. Should I give him teacher rec forms from all the early and reg. colleges? or do I just give him list of all the colleges I'm applyin to and forms for the early colleges?

2) One ED and multiple EAs are possible given that those colleges' policies allow that, right?

3) On common app. there is not space to say about my extracurricular achievements! (awards and etc) where do I put them?

4) One of the teacher asked for my biographical info. one of the questions ask for personal developement. Is it better to say bad thing about me that became good or is it better to say a little bad thing that eventually got better?

5) If I do not waive my right and indicate that I might see the rec in the future, will the adcoms not take the recs seriously?

Thanks!

please help!

Re #1: The teachers may prefer to receive everything at once. Both of the teachers who are writing recommendations for my daughter, who is also applying ED, specified that.

3: I think most people write up a sort of resume. If you're applying on paper, you can enclose it with your application. If you're applying online, your guidance counselor might enclose it with the transcript and other paperwork.

4: It depends on the nature of the problem. For example, if it's a health issue that has now been resolved or if it's a problem that happened before you started high school, you might just want to not mention it at all. On the other hand, if you got arrested six times last summer, you're going to have to talk about it.
