I am a junior in high school and next year I'm looking to apply to UCLA, UCB, UCSB, USC and a few others. I have had serious medical problems since 8th grade that are seemingly starting to go away. In order to explain questionable areas of my application I was wondering if I could send in a doctors note or an explanation of what exactly I was diagnosed with, as it is uncommon.

As far as academics, I have a 4.0 this year but last year was very difficult health wise and I received about a 3.4. My SAT score is 2100+ and even though I have been in and out of school I have held positions in student government, model un, and a community service organization. Also, my hardships have given me inspiration to be a volunteer at a nearby hospital working with kids and recently I have been very dedicated to that.

Besides touching on my obstacles in essays, is there any other way of providing these universities with extra information? Also has any had experience in a relatable situation or known someone who has? How do you think colleges will react to my situation? Thanks!
