Hepatitus B

umm I think I was suppose to fill out some form on the internet about if I got the heppatitus vaccination on april 15th or so...

also I'm not sure if i have gotten it as of right now...lets say i havent
and since the vaccination takes 6 months...

am I boned?
and if I am what should i do to prevent getting the shaft

omg! i didn't do that either. we're supposed to fill out a form??? i didn't even know where i was going to college on april 15th...

you might want to check past immunization (sp) records, i freaked out too at first, but apparently i already got it in 7/8th grade.

and if you haven't probably call up your doctor's office stat and try to get all of the shots scheduled before sep. 20th

as it turns out i think i got it too, but i never put in that form that was due on the 15th...wut i do?

also it demands that i put in my physicians name address phone number and all this ridiculous stuff...

i took the shot seven years ago theres no way i can fill out the freaking form

i thought it was before sept 20...am i wrong?

well the front page tells you april 15 but the actual form tells you september 20. But realistically i don't think they would ask you to turn in all that stuff before decisions are made. So i think the 20th should be the more reliable date.

Okay, if you went to middle school in California odds are you have had the 3-shot Hep B series. It is mandatory to attend CA middle schools.

The reason UCLA puts April 15 as the action date is that the series requires 3 shots, spread out over a number of months. Thus, if you have not had them already, you need to start them NOW! in order to have completed them by the time school starts in mid/late Sept.

Ask your pediatrician for your vaccination record. It's a good thing to have on hand anyway.

crap i need to get mine ASAP. are there any side effects? t the vaccination

^ are you kidding?
you already go here...

well what if we don't have physician's name because I think all I have to the record book with stamps saying I took the shots...

indeed...

i just had to resort to saying it was a school vaccination
so i put my schools address and phone number...they can deal with it cuz there is actually no other way around it....the person who gave the shot scribbled down their name (which needless to say is completely illegible) but otherwise i have NOTHING

ok so if i fill the form out tomorrow ( i dont know my physician's name), am i too late?

never got around to it liyana. any side effects?!??!

^ hell if i remember... i don't even know when i got it!
...so i'd say no xD

where is the website to enter immuniziation dates?

http://www.studenthealth.ucla.edu

calm down, its all good...you can still turn it in, i think

i turned mine in before orientation

i got my vaccination this summer.

there were side effects. but nothing major.

i highly recommend you get it.
