I am kinda confused, some schools say they require "teacher recommendations" whereas some say they require "teacher evaluations"

is there a difference between them?

im guessing the "teacher evaluations" is the common app form and the "teacher recommendations" dont require that form, just the recommendation?

your thoughts would be helpful

Both the exact same thing.

sorry, but do you need or NOT need the forms with the teacher recommendations
sorry for being foolish, but im just not getting it

I'm pretty sure every school wants your teacher to fill out a form in addition to writing a recommendation; some schools just use the common app form (if they're part of the common app) and others have their own form.

I guess the wording they use (some use "evalution", some use "recommendation") might make it confusing, but generally they mean the exact same thing; they want your teacher to fill out the form as well as write a letter of recommendation.

Letters of Recommendation

Tufts requires one letter from a teacher in a junior or senior year major academic course (math, natural science, social science, English, or a foreign language) and one letter from a guidance counselor. Additional letters, though not encouraged, may be submitted if the student feels they can add new perspective to his/her application.

Therefore, no form required?

Since Tufts makes available the Common App Teacher evaluation form to those who want to do a paper application,

I think they expect teachers to use that. If you want details about an individual institution's policy, it's best to ask the school directly, though.
