Guidance counselors at son's school hand out forms for the students to give the teachers and the teachers hand them into Guidance. Has anyone heard of problems with colleges when these types of forms are used vs. the college's own forms?

I don't know of any problems. My school uses these forms also, and I think they're quite effective in aiding guidance counselors. The more information they have, the better their rec will be (and this is the one sent directly to college, not your school's unofficial documents).

I compared it to the Common Apps recommendation form and it's similar, but has 5 less categories to rate - for example, "Maturity" is left off of the HS's form.
