I'm a freshman at a local community college and was wondering how hard it is to get in to JHU? There is absolutely no other school I want to go to; Hopkins is the one. I know their acceptance rate for transfers is less than 10% (last year was 8.5), but what grades are needed to be accepted? Unfortunately I had an injury over a year ago that left me with Post Concussive Syndrome. The effects of it are memory and concentration problems, that have largely affected my performance in school. I know I have to work much harder now for the same grades I could have got before, so it would be great to know ahead of time how much I have to put in.
In high school i had an 88 average overall, most of my lower grades coming from freshman year. I only took two APs, but I was in many clubs, sports, summer programs and had hours and hours of community service. My SATs were only a little above the national average, which is very low to apply to JHU, however transfers don't have to submit scores. I am a chemistry major at the moment.
My question is what grade do I have to get in college to truly have a chance at such a selective school?

My schedule:
Chemistry I
Calculus I
English Composition
Mysteries of Sleep and Dreaming (psych course)
I took precalc over summer online and got a 4.0, so my transcript already has a little boost.
I'm also an active member in the college's radio station and a member in chemistry club, math club, and campus activity board. Ive modeled for the college and have done some community service projects during my one month of attendance.

What could be the possible lowest GPA to get to still have any chance, or does it have to be a 4.0?
Also, what extra curricular activities are necessary to make a great impression towards the college?

Thank you for the help!
