How can I stop screwing up in math?

So I've been taking this SAT prep class for the past few months. It's alright, I guess. Teachers are pretty awesome and chill, we get in a practice test every week, go over our mistakes, etc.

So anyways, for the past 5 tests, I've been always within 800 range for math. The problem? I keep making bloody stupid mistakes. Every time. I have no idea why. It's add a number incorrectly here, misinterpret a graph there, find the mean instead of the median, etc. It's really quite pathetic.

Any tips on stopping this problematic behavior? I know there's just about no curve for math, and missing one or two can drop your score by a good 50 points, which is no good at all if I'm going to go for that 2400 (usually miss like 2 in writing and reading so the curve will hopefully cover that).
