How do fee waivers for applications work?

Do my counselors just write the office of admissions a letter? Does he mail it in separately from the school report?

There is a form that the counselor signs. There are two types of forms - one comes from college board and can also be used to get a waiver of fees for the SATs - see http://www.collegeboard.com/student/testing/sat/calenfees/feewaivers.html
and

The other uses a form provided by National Association for College Admission Counseling (NACAC)
