<p>Is put with my admissions file? I've written it and everything but am wondering what information I need to put on it (like DOB, full name, etc??)</p>
<p>I would head each page with name, DOB and your high school code..the standard common app formation.</p>
<p>Is that necessary, because I just typed my essay, uploaded and then submitted ? I didn't include my personal information in it</p>
