How do I request a LoR to UCB?

So, I just received an email from UCB requesting a letter of recommendation. How exactly do I request a LoR? Do I have to give the teachers a folder about my grades, activities, etc? Or do I just go empty-handed and ask for a letter?

P.S: Does receiving a letter means that I am borderline for admission?

But you can create a brag sheet that list all your ECs achievements etc. to give to your teachers
